Cody Lee Adams, 26, passed away on Monday April 15, 2019 at Jewish Hospital.
Cody is preceded by his mother Brenda and grandfather Paul Herzog.
He is survived by his brother Brandon Smith, sisters Ashley Newton (Seneca), Shelbi Adams, grandmother, Wanda Herzog, nieces, Taylor and Layla. He will also be missed by his aunt and uncle Jess & Laura Herzog and his girlfriend Melissa.
A memorial service will be held Tuesday at 2:00 pm at Louisville Memorial Gardens Funeral Home, 4400 Dixie Highway. Visitation will be from 11:00 am until time of service.
