BOZAK, "BERNYE" (TALLEY), 68, of Louisville, passed away January 3, 2013 at her home.
She was a homemaker and a Baptist by faith.
Survivors include her loving husband of 49 years, Thomas Michael Bozak Sr.; children, Lisa Poole, Angela Baker, Thomas Bozak Jr., and Todd Bozak; grandchildren, Tim Markwell, Todd Wheeler, Shelbie Johnson, Eric and Ashley Bozak and Jonathan, Kelci, Jordan and Sydney Baker; sisters, Esther Adams, Virginia Lucas and Barbara "Bobbye" Grey; and brother, Kenneth Talley.
Funeral service will be held at 1 p.m. Monday at Owen Funeral Home, 5317 Dixie Highway, with burial in Louisville Memorial Gardens, West. Visitation will be 1-8 p.m. Sunday and after 10 a.m. on Monday at the funeral home.
