CARBY, JACQUELINE ANN (DUKE), 74, of Louisville, passed away Thursday, June 3, 2010.
She was retired as a teacher from Jefferson County Public Schools.
She was preceded in death by her husband of over 50 years, Frederick Sherman Carby; and her daughter, Sharon Ann Carby.
Mrs. Carby is survived by her children, Stephanie Phelps (Dan) and Greg Carby (Nancy); grandson, James Frederick Phelps; and great-grandchildren, Alessa Mae Phelps and Samuel James Phelps.
Her service will be 10 a.m. Monday at Owen Funeral Home, 5317 Dixie Highway with burial in Louisville Memorial Gardens, West. Visitation will be 5-8 p.m. Sunday at the funeral home.
Expressions of sympathy may be made to Jewish Hospital Palliative Support Program or VNA Nazareth Home Care.
